Please utilize this form for all prescription refill requests. Please allow up to 72 business hours for refill requests to be completed.

For any unplanned refill request you will be subject to a charge of $25. (Examples of unplanned refills are lost prescriptions, missed appointments due to no shows or not scheduling, leaving a prescription at home during a vacation, etc.) Otherwise, there is not a charge for refill requests outside of an appointment.

For medication changes made outside of an appointment, you may be subject to a fee of $50-$100.
